The template

This form records your authorization for Company name (the "Company") to contact the references you list below, and your former employers, as part of the hiring process for the position of Role applied for. Please read it in full, complete the fields, and sign at the end.

1. What you are authorizing

By signing this form, you authorize the Company and its designated representatives to contact the people listed below, and the former employers reflected in your application, to verify the information you have provided and to ask about your work history and professional performance. Reference conversations may take place by phone, video call, or email, or through a structured questionnaire, at the Company's choice. References are usually contacted late in the process, after interviews, and you will be told before outreach begins.

2. What may be discussed

Topics raised with references and former employers are limited to matters relevant to the role, and typically include:

  • Dates of employment and positions held.
  • Main responsibilities and the scope of your work.
  • Strengths, working style, and collaboration with others.
  • Quality and reliability of your work, including areas for growth.
  • Eligibility for rehire, where the former employer chooses to share it.

The Company does not ask references for information unrelated to the role or for personal details that have no bearing on a hiring decision.

3. Your references

List at least two professional references who can speak to your recent work β€” ideally people who managed you or worked closely with you within the last few years. Tell your references to expect contact; a warned reference responds faster and speaks better.

4. Your current employer

The Company will not contact your current employer without your explicit permission, so applying never puts your existing role at risk. Indicate here whether the Company may contact your current employer and, if so, from what point in the process: Current employer contact permission.

5. Release

You release the references you list, the former employers who respond, and the Company and its representatives from claims arising out of good-faith statements made, requested, or received while checking references under this authorization. This release does not cover statements made in bad faith or known to be false when made.

6. Validity and withdrawal

This authorization applies only to the recruitment process for the role named above and expires when that process ends. You may withdraw it at any time before the checks are completed by writing to your Company contact; the withdrawal will be respected, though the Company may be unable to continue the process without completed references. Information gathered is kept confidential, shared internally only with the people involved in the hiring decision, and handled according to the Company's data practices.

7. Acknowledgment

By signing below, I confirm that the references listed are genuine and may be contacted as described; that the work history I have provided is accurate; and that I understand and accept the scope of this authorization, the release above, and my right to withdraw before the checks are complete.
