The template

This form records a candidate's informed consent for a recruiting firm to store and process their personal data for recruitment. It explains, in plain language, what data is involved, what it is used for, who it is shared with, how long it is kept, and how consent can be withdrawn at any time.

1. Who is asking for consent

The organization requesting this consent (the "Recruiter") is Recruiter firm name. Questions and requests about your data go to: Privacy contact.

2. What data is covered

  • Identity and contact details: name, email, phone, location.
  • Professional background: resume, work history, education, certifications, skills, portfolio or work samples, and public professional profiles you share.
  • Process data: interview notes, assessment results, compensation expectations, availability, and communication history with the Recruiter.
  • References you provide, and feedback received from them.

The Recruiter does not ask you to provide sensitive personal data for recruitment, and you should not include it in your materials.

3. What your data is used for

  1. Evaluating your fit for roles you apply to or express interest in.
  2. Presenting your profile to specific prospective employers — only with your knowledge for each opportunity.
  3. Contacting you about your active processes and, if you agree below, about future opportunities that match your profile.
  4. Meeting the Recruiter's record-keeping obligations for its recruiting activity.

4. Who your data is shared with

Your data is shared with prospective employers for opportunities you are considering, and with service providers the Recruiter uses to run its business (such as applicant tracking and e-signature tools) under contracts that protect your data. It is not sold, and it is not shared with anyone else unless the law requires it.

5. How long it is kept

Your data is kept for Retention period from your last interaction with the Recruiter, then deleted or anonymized, unless the law requires keeping specific records longer or you renew your consent.

6. Your rights

  • Access a copy of the data the Recruiter holds about you, and correct anything inaccurate.
  • Ask for deletion of your data, subject to records the law requires the Recruiter to keep.
  • Withdraw this consent at any time by writing to the contact in section 1. Withdrawal stops future processing but does not make past, lawful processing invalid.
  • Complain to your data protection authority if you believe your data is mishandled.

7. Consent

  1. I, Candidate full name, have read this form and consent to the Recruiter storing and processing my personal data as described above.
  2. Keep-in-touch consent (optional — write yes or no): I also agree to be contacted about future opportunities that match my profile: Future opportunities consent.
  3. I understand this consent is voluntary and that I can withdraw it at any time without giving a reason.
