The template

This acknowledgment records the company property returned by a departing team member, its condition on return, and both parties' confirmation of the handover. Keep a signed copy with the offboarding record.

1. Parties and return date

Employee full name (the "Employee") returned the items listed below to Company name (the "Company") on Return date.

4. Company confirmation

The Company confirms receipt of the items listed above in the condition noted. Any damage or missing accessories discovered during inspection will be raised with the Employee promptly and in writing.
